Dental Policies

To help ensure a long lasting and well-informed relationship, we have listed our policies as they concern you. Please read through the following policy information. Financial Policy

Insurance

As of January 2010, we accept Delta Dental premier plans. With the exception of Delta Dental, we are a fee-for-service provider which means we would expect payment for services on the day of your visit. However, we would be happy to file an insurance claim for you. Just note that any reimbursement would come directly to you and not through our office unless you are a Delta Dental subscriber.

Payment Expectations

  • Upon completion of your dental appointment, the front desk team will produce an invoice outlining the costs. You will be expected to pay for services provided at that appointment.
  • We will also provide a 12 month interest free program through an outside financing company. Credit approval is required.
  • Oral Conscious Sedation appointment - to reserve the time for your appointment, the total fee for each sedation appointment is due at least 2 weeks prior to the appointment. If the treatment changes during the visit and the total cost for the visit changes, a reconciliation of the additional fees or the refund owed will be made at the front desk after your visit.

NSF Checks

If we receive a check returned for insufficient funds, the following will occur:

  • A $20.00 charge will be applied to your account
  • You must clear the account promptly by cash, certified check, money order, or credit/debit card
  • Your privilege to write checks may be jeopardized

Scheduling Policy

Rescheduling or Canceling Appointments- The office requires that you inform us if you need to reschedule.

Appointment confirmation - We will attempt to reach you by email, cell phone text message, or telephone prior to your appointments. We ask that you please reply in some form to let us know you will be making we reserve the right to give your treatment time to another patient.

Missed and Late Appointments- Your appointment time has been reserved especially for you at exclusion of others who may be waiting for an appointment. If you miss your appointment and we do not receive adequate prior notice, we will have no time to fill your space. We therefore reserve the right to charge for missed appointments. If you should arrive late, we may not be able to see you for that appointment. Missed Appointment Fees (2010): Doctor’s Fee $118.00 per hour Hygienist’s Fee $54.00 per hour
